Ankle and Feet Pain Conditions

Achilles tendonitis

The achilles tendon is prone to develop tendinitis at the level it connects the calf musculature to the heel bone. This is a challenging condition to manage for the patients and clinicians. This rise to considering long-standing morbidity, hence, early intervention is suggested.

Symptoms:

• Morning stiffness and pain after prolonged rest.
• Palpable /visible lump over the middle of the tendon
• Pain, and stiffness, along the swollen part of the Achilles tendon.
• Feeling weak Achilles tendon/calf muscles.

Who is prone to develop this:

• Obese people
• A sudden increase in the distance of running /walking.
• Tight calf/gastrocnemius/hamstring muscles and Achilles tendon.
• Starting new activities/hobbies: Running, walking, hiking, mountain climbing, sprinting and jumping.

How can we help you with this?

If you have tried, rest, ice, pain medication, and physiotherapy or if you are struggling to cope with the physiotherapy: the following special procedures can be offered at our clinics.

High volume Injection (HVI):
Prior to the HVI special procedure for the Achilles tendon, we carry out a diagnostic ultrasound scan to confirm Achilles tendinitis and rule out other problems.
HVI is a special procedure for Achilles tendinitis. There is no steroid in this injection, due to the high risk of Achilles tendon rupture.

A large volume of local anaesthesia and saline water is injected in between the tendon and its sheath called paratenon. Since the Achilles tendon is just under the skin there is not much needle pain to experience. We use a large amount of local anaesthesia to make the whole procedure comfortable. This suppresses the new blood vessel formation to control inflammation and promotes the natural healing process.

The pain relief HVI procedure can last from weeks to several months. During this period we strongly suggest a course of physiotherapy to strengthen the Achilles tendon. Some patients may need repetition of this procedure after a few months to years.

Platelet-rich platelet Injection (PRP):

This popular relatively new treatment is effective in promoting natural collagen synthesis, thus enhancing the natural healing mechanism of the tendon. This is achieved by injecting the PRP, which contains highly concentrated growth factors.

You may need to complete a course of PRP treatment, which consists of 3 injections with 3-6 weeks intervals in between each injection. We recommend you consider 3-4 weeks of relative rest with no exercises or strenuous activities after each injection.

PRP preparation: Upon collecting a small amount of blood from the patient’s vein, PRP is separated using a gravitational centrifuge in the clinic.

Plantar Fasciitis

Plantar fasciitis is the most common condition which results in heel pain. You could experience a gradual or sudden onset of heel pain due to irritation /inflammation of the plantar fascia ligament in your foot.  Foot pain, which is worse in the morning or after rest, is typical with plantar fasciitis.

What factors increase your risk of developing this condition?

  • Being overweight
  • Running/walking
  • Wearing poorly fitted shoes
  • Having naturally flat feet
  • Prolonged standing jobs/hobbies
  • Tight hamstrings/calf/gastrocnemius muscles

The following should consider as the first line of treatment:

  • Weight loss
  • Night splints
  • Proper footwear
  • Cross frictional massage
  • Regular lower limb stretches
  • Rest, ice packs, stretching exercises

If the above treatments fail to control the symptoms you may consider the following ultrasound-guided injections at our clinic:

Are ultrasound guided injections painful?

Discomfort from the needle insertion can be minimised using local anaesthesia using a very fine small needle. Ram is experienced and has carried out more than 30,000 Ultrasound guided injections so far. He routinely treats complex patients and patients with needle phobia, through the technique he developed through his experience.

What is an ultrasound guided cortisone / Steroid / Corticosteroid injection?

A cortisone injection is also known as a Corticosteroid injection or steroid injection. This anti-inflammatory strong medication is used for many inflammatory and arthritis conditions to control pain, swelling, and stiffness and improve function. Corticosteroid injections are NOT the same used by bodybuilders use. Corticosteroid injections do not cause weight gain.  To find out more please visit our ultrasound-guided steroid injections page from the ‘Types of Guided injection’ menu on the top.

Always, consider having the Steroid injections under ultrasound guidance for accuracy.

What is an ultrasound guided hyaluronic acid (HA) injection?

HA injection is also known as Gel injection or Lubricant injection. This viscosupplementation injection is a natural component of synovial fluid. This tends to increase joint fluid viscosity and restores joint lubrication. Hence, it acts as a shock absorber to control joint/tendon symptoms. Limits further damage to the joint and delays or avoids the need for surgery. Also, helps to enjoy your daily activities or your preferred hobbies. To find out more please visit our  GUIDED ULTRASOUND HYALURONIC ACID INJECTIONS page from the ‘Types of Guided injection’ menu on the top.

These injections are very popular, especially for knee joint pain. Any number of injections can be taken in a given period, due to minimal or no side effects from these injections. Usually, a course of 3 Injections is preferred, though some may do well just with an injection per year 9 we have patients managing just with 1/year.

What is an ultrasound guided PRP injection?

Platelet Rich Plasma (PRP) injections are rich in growth factors (help repair the cell and its growth) and prepared from the patient’s blood. Separated growth factors from the patient’s blood are injected back into the affected area of the same patient. PRP’s ability to heal and even regenerate injured or ageing tendons, ligaments and joints, provides some patients with an alternative to surgery. PRP therapy may reduce the required healing time for the injuries ( especially for sports injuries and early Arthritis ); otherwise, athletes/ sports professionals may require longer periods of rest from their sports-related activities.  To read more please visit our Ultrasound Guided PRP page.

Always, consider having the PRP injections under ultrasound guidance for accuracy.

can i drive after an injection

You will not be able to drive immediately after the procedure. Bring someone with you Or use another form of transport to return home. This is mainly due to your driving licence policy, rather injection efficacy. Most motor insurance policies will not allow you to drive for 3-5 hours after the injection with Local Anesthesia.

What should i expect after an injection?

After the injection, you may feel pain-free for a couple of hours due to Local anaesthesia. However, the pain returns after 2 hours, and you may even feel more pain for a couple of days due to bruising pain. Usually, you should start feeling better from the third day onwards. Pain relief from the steroid injection may take 1 to 2 weeks.

How to manage bruising pain?

Discomfort from the needle insertion can be minimised by using an ice pack (wrapped in a towel, to minimise the risk of frostbite from direct skin contact) at the treatment area. 3-4 times a day and 10-15 minutes at each time.

Will i always feel 100% pain free?

We can accurately inject the treatment area. However, pain relief depends on several factors, such as the severity of the problem, body response to the steroid injection, rest period following an injection,  rehabilitation and exposure to the risk factors.
