Common Causes of Ankle and Feet Pain
Achilles tendonitis
The Achilles tendon is susceptible to tendinitis at the point where it connects the calf musculature to the heel bone. This is a challenging condition to manage for the patients and clinicians. When considering long-term morbidity, we recommend early intervention.
Symptoms:
• Morning stiffness and pain after prolonged rest.
• Palpable/visible lump over the middle of the tendon
• Pain and stiffness along the swollen part of the Achilles tendon.
• Feeling weak in the Achilles tendon/calf muscles.
Who is prone to developing this?
• Obese people
• A sudden increase in the distance of running or walking.
• Tight calf/gastrocnemius/hamstring muscles and Achilles tendon.
• Starting new activities or hobbies: running, walking, hiking, mountain climbing, sprinting and jumping.

High Volume Injection (HVI):
Before the HVI special procedure for the Achilles tendon, we carry out a diagnostic ultrasound scan to confirm Achilles tendinitis and rule out other problems. HVI is a special procedure for Achilles tendinitis because is no steroid in this injection due to the high risk of Achilles tendon rupture.
We inject a large volume of local anaesthesia and saline into the space between the tendon and its sheath, known as the paratenon. Since the Achilles tendon is just under the skin, there is not much needle pain to experience. We use a large amount of local anaesthesia to make the whole procedure comfortable. This suppresses the new blood vessel formation to control inflammation and promotes the natural healing process.
The pain relief from the HVI procedure can last from weeks to several months. During this period, we strongly suggest a course of physiotherapy to strengthen the Achilles tendon. Some patients may need repetition of this procedure after a few months to years.
Plantar fasciitis is the most common condition that results in heel pain. You could experience a gradual or sudden onset of heel pain due to irritation or inflammation of the plantar fascia ligament in your foot. Foot pain that is worse in the morning or after rest is typical of plantar fasciitis.
What factors increase your risk of developing this condition?
- Being overweight
- Running/walking
- Wearing poorly fitted shoes
- Having naturally flat feet
- Long-standing jobs or hobbies
- Tight hamstrings/calf/gastrocnemius muscles
Consider the following as the first line of treatment:
- Weight loss
- Night splints
- Proper footwear
- Cross-friction massage
- Regular lower limb stretches
- Rest, ice packs, and stretching exercises.

How long does an ultrasound guided injection take?
This will vary depending on the area and other factors, but an assessment, scan & ultrasound-guided injection will generally take around 60 minutes. Ultrasound-guided injection alone is done in a few seconds. Injection alone is carried out in a few seconds.

What is an ultrasound guided cortisone / Steroid injections?
A cortisone injection is also known as a Corticosteroid injection or steroid injection. This is a strong anti-inflammatory medication that is used for many inflammatory and arthritis conditions to control pain, swelling, stiffness and improve function. Corticosteroid injections are NOT the same used by bodybuilders. Corticosteroid injections do not cause weight gain. What is an ultrasound guided hyaluronic acid (HA) injection?
HA injection is also known as Gel injection or Lubricant injection. This viscosupplementation injection is a natural component of synovial fluid. This tends to increase joint fluid viscosity and restore joint lubrication. Hence, acts as a shock absorber to control joint/tendon symptoms. Limits further damage to the joint and delays or avoids the need for surgery. Also, helps you enjoy your daily activities or your preferred hobbies. These injections are very popular, especially for knee joint pain. Any number of injections can be taken in a given period, due to minimal or no side effects from these injections. Usually, a course of 3 Injections is preferred, though some patients do well just with an injection per year 9.
Can i drive after an injection
You will not be able to drive immediately after the procedure. Bring someone with you or use another formof transport to return home. This is mainly due to your driving licence policy, rather than injection efficacy. The majority of motor insurance policies will not allow you to drive for 3-5 hours after the injection with Local Anesthesia.

What should i expect after an injection?
After the injection, you may feel pain-free for a couple of hours due to Local anaesthesia. However, pain returns after 2 hours; you may even feel more pain for a couple of days due to bruising. Usually, you should start feeling better from the third day onwards. Pain relief from the steroid injection may take 1 to 2 weeks.

How to manage bruising pain?
Discomfort from the needle insertion can be minimised by using an ice pack (wrapped in a towel, to minimise the risk of frostbite from direct skin contact) on the area of treatment. Use the ice pack 3-4 times a day for 10-15 minutes each time.
