Common Causes of shoulder pain
Shoulder injuries are common in sports such as tennis, swimming, baseball, weightlifting, rugby, cricket, squash, and any other sport demanding a quick change in pace and direction. Our clinic routinely diagnoses and treats the following shoulder disorders:
- Rotator Cuff Injuries
Surrounding the shoulder joint, the rotator cuff is a collection of muscles and tendons that provide support and aid movement. Particularly prone to rotator cuff injuries are sports such as tennis, swimming, baseball, and other activities requiring repeated overhead motions. - Labral Tears
The labrum, a ring of cartilage around the shoulder socket, serves to stabilise the joint. Sports such as weightlifting, rugby, and football, which require quick, strong shoulder motions, can cause labral tears. - Tendinitis
Overuse often causes tendinitis, the inflammation of a tendon. Popular therapeutic choices with rehabilitation are PRP, steroids, and viscosupplementation (gel) injections. - Bursitis
The bursa is a small sac that cushions several shoulder tissues to reduce friction. Bursitis is the bursa’s inflammation. The subacromial bursa and the subdeltoid bursa are two common bursae that are sensitive to inflammation in the shoulder. You can diagnose the bursa and aspirate the fluid under ultrasonic direction.
Sports or careers: Requiring repetitive shoulder movement can cause overuse conditions, including tendinitis or bursitis.
Poor posture: Over time poor posture can lead to misalignment and increased tension in the shoulder joints, resulting in pain and injury.
Sudden collisions: Falls or accidents could cause dislocations, rotator cuff tears, or fractures.
Weak joint mechanics: Caused by weak or unbalanced shoulder muscles increase the risk of strain and other complications.
Osteoarthritis: This is an example of age-related wear and tear, which can gradually weaken the shoulder joint and cause limited mobility and chronic pain.
Arthritis is a common cause of shoulder pain and dysfunction, significantly impacting your daily activities and quality of life. The following are the various common arthritic conditions observed in the shoulder:
- Osteoarthritis
The most common type of arthritis, osteoarthritis, is characterised by the gradual wear and tear of the shoulder joint cartilage. This leads to reduced mobility, discomfort, and stiffness. - Rheumatoid Arthritis
An autoimmune disease, rheumatoid arthritis, causes inflammation of the shoulder joint lining. This inflammation may damage the joint cartilage and bones, leading to discomfort, swelling, and potential abnormalities. - Traumatic Arthritis: Post-Trauma
Following a shoulder injury, including a fracture or dislocation, post-traumatic arthritis can develop. Arthritis can be caused by cartilage damage long after the injury heals. - Arthropathy of the Rotator Cuff
The condition is caused by a long-standing rotator cuff injury that worsens arthritis in the shoulder. Damage to them may lead to severe arthritis and joint issues.

How long does an ultrasound guided injection in the shoulder take?
This will vary depending on the area and other factors, but an assessment, scan & ultrasound-guided injection will generally take around 60 minutes. Ultrasound-guided injection alone is done in a few seconds. Injection alone is carried out in a few seconds.
Are ultrasound guided injections painful?
Discomfort from the needle insertion can be minimised by using local anaesthesia using a very fine small needle. Ram is experienced and carried out more than 30,000 Ultrasound-guided injections so far. Who is more prone to Shoulder pain?
Shoulder pain can be experienced by any age group, occupation or sport. Occupations involving repetitive movements, such as overhead activities, lifting, pulling, pushing, gym-related injuries such as weight training and sports injuries from running, rugby, football, cricket, golf, hockey, jogging, running, cycling, etc. prone to develop shoulder problems.
The elderly population is prone to either arthritis or degenerative conditions. Traumatic rotator cuff tears following a fall or while lifting a heavy object are also possible. What is an ultrasound guided cortisone / Steroid / Corticosteroid injection?
A cortisone injection is also known as a Corticosteroid injection or steroid injection. This is a strong anti-inflammatory medication used for many inflammatory and arthritis conditions to control pain, swelling, and stiffness, and improve function. Corticosteroid injections are NOT the same used by bodybuilders use. Corticosteroid injections do not cause weight gain. What is an ultrasound guided hyaluronic acid (HA) injection?
HA injection is also known as Gel injection or Lubricant injection. This viscosupplementation injection is a natural component of synovial fluid. This tends to increase joint fluid viscosity and restores joint lubrication. Hence, acts as a shock absorber to control the joint /tendon symptoms. These injections are very popular, especially for knee joint pain. Any number of injections can be taken in a given period, due to minimal or no side effects from these injections. What is an ultrasound guided PRP injection?
Platelet Rich Plasma (PRP) injections are rich in growth factors (help in repairing the cell and its growth) and prepared from the patient’s own blood. Separated growth factors from the patient’s blood are injected back into the affected area of the same patient. PRP’s ability to heal and even regenerate injured or aging tendons, ligaments, and joints, provides some patients with an alternative to surgery. PRP therapy may reduce the required healing time for the injuries (especially for sports injuries and early Arthritis); otherwise, athletes/sports professionals may require longer periods of rest from their sports-related activities. can i drive after an injection
You will not be able to drive immediately after the procedure. Bring someone with you Or use another form of transport to return home. This is mainly due to your driving license policy, rather than injection efficacy. The majority of the motor insurance policies will not allow you to drive for 3-5 hours after the injection with Local Anesthesia.

What are the common shoulder problems you can help?

Rotator cuff tendinitis, impingement or Bursitis problems: Often painful upon using your shoulder/arm, lying on the affected shoulder or with repetitive movements. Pain can be an intense sharp shooting pain with a particular movement.
Frozen shoulder /Osteoarthritis/rotator cuff tear: Often present as stiff and painful shoulder movements. Struggles with daily activities. Night sleep is affected.
Acromioclavicular (AC) joint Problems: Painful over the top of your shoulder. Painful with overhead activities and reaching onto the opposite shoulder. This could be due to arthritis, ligament sprain, joint subluxation or displacement.
Tingling, pins, and needles to the hand: This could be due to nerve pain radiating from your neck.

What should I expect after a Shoulder Injection?
After the injection, you may feel pain-free for a couple of hours due to Local anaesthesia. However, the pain returns after 2 hours, you may even feel more pain for a couple of days due to bruising pain. Usually, you should start feeling better from the third day onwards. Pain relief from the steroid injection may take 1 to 2 weeks.
How do I manage bruising pain?
Discomfort from the needle insertion can be minimised by using an ice pack (wrapped in a towel, to minimise the risk of frostbite from direct skin contact) to the area of treatment. Use the ice pack up to 3-4 times a day and 10-15 minutes at each time.
Will i always feel 100% pain free?
We can accurately inject the treatment area. However, pain relief depends on several factors, such as severity of the problem, body response to the steroid injection, rest period following an injection, rehabilitation and exposing to the risk factors.
how many shoulder injections can i receive?
Corticosteroid Injections: 3 or 4 per treatment area/year.
Hyaluronic Acid: No limit as long as you are benefiting.
Platelet-rich plasma (PRP): This is a course of 3, which you can repeat as you like.

It is hard to determine without an assessment. It may be possible to estimate after assessing the patient. Usually, lasts up to 3 months. There are many patients who benefited for more than a year to 2 years.
